The Client Profile Form is used to help assess if the Farm is an appropriate environment for your dog. For the health and safety of your dog and others staying at the Farm, please answer all questions as accurately as possible. Once completed, this form will be kept on file at the Farm. Client Profile Form (in Adobe PDF format) Client Profile Form (online submission form) The Check-In Form contains information that may change with each of your dog's visits to the Farm (for example, where you may be reached while away), so this form should be completed when you drop your dog off. Check-In Form (in Adobe PDF format) The Lucky Dogs Farm Terms and Conditions agreement needs to be completed and signed prior to your dog's first stay at the Farm.
